The Student Community Service Award recognizes a student member chapter that has demonstrated outstanding community service activities.

GUIDELINES

Award recipient will be notified in May 2017 after judges review has been completed. Presentation to the award recipient will be made at ADHA’s at the 94th Annual Conference in Jacksonville, FL, during the Saturday General Session on June 17, 2017.

DESCRIPTION

This award is presented to a student member chapter that has demonstrated outstanding community service activities.

AWARD

If your submission is selected you will receive the following award benefits:

  • $3,000 cash award
  • An engraved award
  • Recognition in Access Magazine, ADHA's official publication
  • Recognition at ADHA's 94th Annual Conference in Jacksonville, FL, during Saturday's General Session on June 17, 2017

JUDGING

The Committee on Student Relations selects one winner per year for this prestigious award. Committee decisions will be final.

APPLICATION CHECKLIST

  • Complete application cover page
  • Complete seven sections of application to describe your project
  • Complete the roster listing all students in your class
  • Save a copy of the application to your computer
  • Print a copy of application for your records
  • Submit application by March 1, 2017

Please note, this application must be completed by a current student member. Additionally, ninety percent (90%) or more of the students in your class must be student members of ADHA. For the roster, please provide student membership ID numbers. Students may locate their ID number on their membership cards or by contacting ADHA Member Services at (312) 440-8900.
